Athletic Eligibility 

All new and returning athletes must be certified annually to be eligible to practice or compete with any teams at Bayside Academy. Beginning with the 2018-19 school year, the majority of the required forms will be completed online through the AHSAA document eligibility program Dragonfly. The AHSAA releases these documents in late May of each year and will be linked to this page for all current and incoming student-athletes in grades 7-12 to complete.

Annual required documents include:
1. Pre-participation Physical Evaluation (completed by student and parents)
2. Physical Evaluation (completed by Medical Doctor)
3. AHSAA Participation Release
4. AHSAA Concussion Information
5. Bayside Academy Consent for Care

Other documents required to be turned in once:
1. Birth Certificate
2. NFHS Sportsmanship Certificate - course must be taken online at 


Encore Sports provides Bayside Academy with an athletic trainer onsite for all practices and home competition. All students are required to turn in an athletic physical and complete a baseline concussion test prior to training with any of our teams. For convenience, Bayside partners with various local physicians to provide athletic physicals at a very low cost in January each year. If a student chooses not to receive a physical exam at this time, the student is responsible for providing the school with a current physical completed by a medical doctor. Physicals are good for one calendar year. 

STUDENT Supplemental Insurance

 All Bayside students are eligible for a supplemental insurance program that will reimburse up to $1000 per occurance for each accident sustained during the calendar year beginning August. 1. For more information please download the registration information and return it to Human Resources.
